What it is
Isotretinoin is an oral retinoid, the molecule sold as Accutane. It shrinks sebaceous glands, cuts sebum output, normalizes how skin cells shed inside the pore, and starves the bacteria that drive inflammation. It is the only acne medication that hits all four causes at once, which is why it is the one drug that can produce lasting remission. The AAD 2024 guideline recommends it for severe acne, scarring acne, or acne that failed standard therapy.
Dosing is weight based: 0.5 to 1.0 mg/kg per day, taken with food, toward a cumulative target of 120 to 220 mg/kg over roughly 4 to 8 months. Going past 220 mg/kg adds side effects without cutting relapse further. The honest relapse math: in a cohort of 19,907 patients, 22.5% relapsed after a standard course and 8.2% needed a second one. Higher cumulative dose meant lower relapse.
California allows prescribing by telehealth under B&P 2242(a), and isotretinoin is not a controlled substance, so the visits are video. What stays physical: iPLEDGE requires pre-treatment pregnancy tests in a medical setting for patients who can become pregnant, and our Torrance office covers both. Labs follow the AAD 2024 minimal schedule, ALT and triglycerides at baseline and once at peak dose. Since August 2026, monthly pregnancy tests can be done at home with prescriber permission.

You enroll in iPLEDGE (mandatory and federal), get ALT and triglycerides drawn at any lab draw site, and, if you can become pregnant, complete the two required pregnancy tests at our Torrance office since iPLEDGE requires a medical setting for those.

Each prescription is a 30-day supply with no refills, filled at an iPLEDGE-enrolled pharmacy, inside a 7-day window for patients who can become pregnant.

Risks, stated plainly
Honest list, not fine print. Every one of these is part of the consent conversation at your video visit.
- Dry lips in nearly everyone: cheilitis affected 93.8% of patients on conventional dosing in trial data, so balm starts day one.
- Dry skin, dry eyes, and nosebleeds are common and track the daily dose.
- Sun sensitivity is real; you burn faster, and sunscreen is required for the whole course.
- Triglycerides and liver enzymes can rise, which is exactly why ALT and triglycerides get checked at baseline and peak dose.
- Mood: the evidence is contested, but depression, anxiety, and suicidal ideation are asked about at every monthly visit, and you should report changes early.
- Pregnancy is absolutely prohibited at any dose; isotretinoin causes severe birth defects.
- iPLEDGE is mandatory and federal: prescriber, patient, and pharmacy all enroll, with no exceptions.

- What does the whole program cost?
- The prescribing visit is $79, follow-up visits are $39, and monthly isotretinoin monitoring is $49 per month, all itemized on the pricing page; nothing is added at checkout. The capsules are paid at your pharmacy, not to us, and generic isotretinoin is inexpensive with a coupon.
- Do I ever have to come in?
- Prescribing is legal fully online in California under B&P 2242(a). If you can become pregnant, iPLEDGE requires the two pre-treatment pregnancy tests in a medical setting, and our Torrance office covers both. Monthly tests can be done at home since the August 2026 iPLEDGE change, and blood draws happen at any local lab.
- Will it affect my mood?
- The population-level evidence is mixed, but the AAD 2024 guideline advises monitoring for depression, anxiety, and suicidal ideation during treatment. We ask at every monthly visit, we want to hear about changes early, and stopping the drug is always on the table.
- Can I get laser or RF microneedling during the course?
- The 2017 ASDS consensus found no reason to delay superficial peels, vascular lasers, or nonablative lasers during or right after isotretinoin under physician judgment. Fully ablative resurfacing waits about 6 months. RF microneedling postdates that consensus, so it is a physician-discretion call made at your in-person exam.
- What if I can get pregnant?
- The rules are federal and absolute: two negative pregnancy tests before the first prescription, two forms of contraception at the same time or committed abstinence, monthly pregnancy tests during treatment, a final test after stopping, and a 7-day window to fill each prescription. There are no exceptions at any dose.
- How dry is dry?
- Plan for it. Cheilitis hit 93.8% of patients on conventional dosing in trial data, so cracked lips are close to universal without a thick balm from day one. Dry skin, dry eyes, and nosebleeds are common; moisturizer, artificial tears, and skipping elective waxing get most people through comfortably.
