Clascoterone (Winlevi)

A topical androgen blocker for acne that works in men and women. Prescribed online in California.

What it is

Acne runs on androgens. Dihydrotestosterone (DHT) binds androgen receptors in sebocytes, the cells of your sebaceous glands, and the glands answer with more sebum and more inflammation. Clascoterone is the first topical that blocks that receptor directly: it competes with DHT inside sebocytes and hair follicles, cutting the oil signal at its source. You apply a thin layer twice daily, and the label covers ages 12 and up.

Until clascoterone, androgen blocking meant oral spironolactone, which is not prescribed to men. Because clascoterone acts locally in the skin, men and women can both use it. In pooled phase 3 trials with 1,421 participants, 19.8% reached clear or almost clear skin at week 12 versus 7.8% on the vehicle cream, an odds ratio of 2.9 (p<0.0001). That sounds modest, but the bar (IGA success) is strict, and no other topical hits this pathway at all.

The AAD 2024 guideline gives clascoterone a conditional recommendation and is explicit that cost, not efficacy, is the limiting factor: Winlevi has no generic. It pairs logically with a topical retinoid because the mechanisms are complementary. The retinoid unclogs the follicle while clascoterone turns down the oil feeding it. Straight answers

How is this different from spironolactone?
Same target, different route. Spironolactone blocks androgens through your whole body as a daily pill; clascoterone blocks them in the skin where you apply it, which is why men can use it. The label did report HPA-axis suppression under maximal-use testing that resolved on stopping, so local is mostly, not perfectly, true.
Can men use it?
Yes. It is the one androgen-targeting acne treatment available to men, because oral anti-androgens like spironolactone are not prescribed to them.
Can I combine it with tretinoin or another retinoid?
Yes, and it is a logical stack: the retinoid normalizes the clogged follicle while clascoterone cuts the sebum feeding it, so the mechanisms do not overlap.
What are the side effects?
Local redness and itching are the common ones. Under maximal-use conditions the label reports reversible HPA-axis suppression that resolved after the cream was stopped.
